DENTAL
DENTAL PLANS
The Chefs' Warehouse will continue to offer dental plans through Cigna and all members will have access to the Cigna network of dentists with two plan options: CIGNA DPPO, and CIGNA DHMO. Diagnostic and Preventive Care are covered at no cost to you. (NEW!) Starting 10/1/2024, both Cigna Dental plans cover Composite (White/Tooth Colored) Fillings. This is covered under the Basic Services.
Please note, the PPO plans allow you to have the flexibility to receive dental treatment from any dentist you choose, either participating or non-participating. However, you will experience greater cost savings by utilizing in-network dentists. To search for a dentist online go to www.cigna.com. DHMO enrollees must select a Primary Care Dentist and there is no coverage for out-of-network services.
Register at www.mycigna.com to access a personalized self-service site for members to access benefit plan information, claim status, EOB history and locating a network provider.
Plan Highlights:
*For services provided by a non-network dentist, Cigna Dental will reimburse according to the Maximum Reimbursable Charge. The MRC is calculated at the 90th percentile of all providers submitted amounts in the geographic area. The dentist may balance bill up to their usual fees.

The Chefs' Warehouse’s vision insurance through EyeMed entitles you to specific eye care benefits. Our policy covers routine eye exams, and provides specified dollar amounts or discounts for the purchase of eyeglasses and contact lenses.
