The Chefs' Warehouse is pleased to announce the Supplemental Benefit options for the 2024-2025 plan year:

  • (NEW!) Hospital Indemnity will be offered through MetLife.
  • Universal Life administered by TransAmerica.
  • Voluntary Legal Protection administered by LegalShield.
  • Pet Insurance administered by Nationwide.

These voluntary benefits offer you the opportunity to provide financial protection for yourself and your family through the convenience of post-tax payroll deductions.

You may choose these plans with or without participation in any other group plan.

(NEW!) HOSPITAL INDEMNITY

Hospital Indemnity insurance is designed to help provide financial protection for covered individuals by paying a benefit due to a hospitalization.

Universal Life coverage provides protection in addition to your company paid life benefits. Once you enroll in Universal Life coverage, your premium rate will be locked in and remain the same for long as you keep the policy active.

LONG-TERM DISABILITY

Voluntary Long-Term Disability Insurance replaces a portion of your income to help make ends meet if you are totally disabled due to a covered accident or covered sickness. This coverage is in addition to your employer-sponsored Short-Term Disability coverage.

All late entrants will be required to complete an EOI document in order to be approved for coverage. You should be prompted to complete EOI in Ceridian Dayforce.
