BASIC LIFE AND AD&D

All full-time employees working 30+ hours per week are automatically enrolled in the Company Paid Basic Life and AD&D Insurance coverage. Please remember to review and confirm your beneficiary information annually.
According to federal law, only the first $50,000 of employer-paid life insurance is not taxable. Premium paid by Weaber, Inc. for coverage levels over $50,000 will be taxable to you and will be included on your year-end W-2 statement.
VOLUNTARY LIFE AND AD&D

In addition to the employee-paid Basic Life and AD&D that Weaber provides, employees may choose to purchase Voluntary Life and AD&D coverage for additional financial protection. Employees may also elect Voluntary Life and AD&D for their spouse and/or dependent children.
Enrollments are subject to Evidence of Insurability (EOI). The EOI form can be found in Employee Navigator. Plan costs can also be found in Employee Navigator during the enrollment process, as costs vary based on age and benefit amount. Please remember to review and confirm your beneficiary information.
SHORT-TERM DISABILITY (STD)
The Hartford
All full-time employees are automatically enrolled in a Company Paid Short-Term Disability plan. This policy provides income replacement should you miss work more than 14 consecutive days due to an injury or sickness. The weekly benefit amount is the lesser of $5,000 or 60% of your basic weekly earnings and, based on disability status, can be paid out for up to 24-weeks.
LONG-TERM DISABILITY (LTD)

New this year, Weaber, Inc. will offer employees the opportunity to enroll in Voluntary Long-Term Disability (LTD) coverage. These benefits help replace income for those disabled longer than the short-term disability period.
This plan provides up to 60% of your basic monthly earnings to a maximum of $6,000. The minimum monthly benefit amount you may elect is the greater of $100 or 10% of basic monthly earnings.
Please note that while this benefit is being offered during Open Enrollment, at least 10% of the Weaber population that is eligible for benefits must participate in order for the benefit to become effective January 1, 2025. If minimum participation is not met, anyone who elects this coverage will not be enrolled and will not have payroll deductions taken.
