NEW FOR 2025!

Hapag-Lloyd is excited to announce that we will be offering our 2025 Medical/Rx plan through Anthem Blue Cross Blue Shield. You will continue to have the option to choose from two medical plans now offered through Anthem or you may elect to opt out of coverage. While the available plans cover the same types of medical services, each provides coverage at a different level and may require you to contribute a different amount per pay period toward the premium. Each medical plan option is summarized below.

OPTION 1: ANTHEM CHOICE CORE PLAN

The Anthem Choice Core Plan allows you and each covered family member flexibility when you have to make health care decisions. You may visit an Anthem health care provider or any provider of your choice. This plan includes in-network and out-of-network deductibles that are lower than those in the High Deductible Health Plan. After you meet these deductibles, your out-of-pocket cost will either be in the form of a copay or coinsurance. Some services have a copapy prior to your deductible being met. Your paycheck contributions will be higher under the Core Plan.

The Core Plan’s highlights are listed below: - You may visit an in-network or out-of-network provider. - When you visit an in-network provider you pay less for covered services. - You must satisfy an in-network and out-of-network deductible before the plan begins to pay for certain services.

- Some services have a copay regardless of deductible being met

OPTION 2: ANTHEM HDHP WITH HSA

The Anthem High Deductible Health Plan (HDHP) works similarly to the Core Plan, allowing you to see your choice of in-network or out-of-network providers, and paying a percentage of coinsurance after you have satisfied your deductible. The HDHP, however, features a higher deductible that must be satisfied before the plan will begin to pay benefits. It also allows you to enroll in a Health Savings Account to be used to pay for eligible medical expenses (including your deductible and coinsurance).

The HDHP features are listed below: - Lower paycheck contributions. - When you visit in-network providers, you pay less for covered services. - You are eligible to enroll in the tax-deferred Health Savings Account. - You must satisfy a deductible before the plan begins to pay for certain services. Even services such as a visit to your PCP or filling certain prescriptions are subject to the deductible.

ANTHEM TRANSITION - THINGS TO KNOW

Hapag-Lloyd wants the transition to Anthem BCBS to be as smooth as possible for all team members. Please utilize the information on this page to assist you and your family during the transition process. If you are in the middle of medical treatments or want to make sure that your providers have the correct information for when you do need care, please follow the steps below. There is more detailed information and instructions on these steps throughout this page.

1. Register for your Anthem member portal using your new Member ID located on your new Anthem ID card

- All employees who enroll in a medical plan will be receiving a new Anthem ID card in the mail

2. Determine if your current provider(s) are In-Network with Anthem

- Use the Anthem Find a Doctor tool to search your current provider(s)

- Contact your provider directly and confirm they accept the Anthem Blue Open Access POS Network for those in GA or the Anthem National PPO (BlueCard PPO) Network for all other states

4. Obtain Refills - Prior to the end of the year, we would recommend obtaining any refills needed on medications from your doctor to ensure that you have some buffer room during the transition.

5. Download your Digital ID card through the Sydney app and share your new Member ID Number with your provider(s)

6. Confirm that your prescription drugs are covered under the Anthem National Direct Plus Formulary

- Consider setting up Mail Order through CarelonRx Pharmacy

- If you are currently receiving mail order prescriptions through CVS Caremark or your retail pharmacy, you will need to reapply with CarelonRx Pharmacy

ANTHEM MEMBER PORTAL

As a Anthem member, you and your dependents will have access to the Anthem member portal. Through the portal you can check the status of a claim, view your Explanation of Benefits (EOB), request temporary and physical ID cards, locate providers, check Rx history, access health reference tools, and more.

  • Click here to access the Anthem member portal registration page
  • Select Member ID
  • Enter your Member ID located on your new Anthem ID card along with your first name, last name, and DOB
  • Continue as prompted until your account set up is complete
