This guide was designed to answer some of the basic questions you may have about your benefits. Please read it carefully along with any supplemental materials you receive.

Please note that this year's open enrollment is PASSIVE, meaning that all current benefit elections will automatically carry over into the 2025-2026 plan year. If you wish to make changes to your previous benefits, add new ones, waive coverage, or change contributions to your HSA for 2026-2027, you must sign into our benefits platform, Workday. Please refer to the Enrollment Information page for instructions on how to enroll.

BENEFITS OVERVIEW

MEDICAL PLAN Onvo will continue to offer employees the option to enroll in one of three medical plan offerings through Highmark. There will be no changes to the medical plan designs. Employee premiums will increase slightly.

SMITHRX New this year, prescription drug coverage will be offered through SmithRx. For the November 1, 2026 plan year, all members will receive new Highmark ID cards that include SmithRx prescription drug information. Your Highmark Member ID will stay the same, no matter which plan you choose. If you need care after November 1st and before your physical ID card arrives, your provider can use your current Highmark Member ID to confirmyour coverage.​

RX N GO Employees may elect to have certain maintenance medications delivered straight to their door in 90-day supplies for a $0 copay and $0 delivery fees! Navigate to the Rx 'N Go page to learn more about the program and confirm whether your prescription(s) are covered.

PATIENT ASSISTANCE PROGRAMS (PAP) Did you know that some pharmaceutical manufacturers may sponsor programs to help individuals under certain income thresholds to receive their specialty medications at no cost? This includes expensive drugs like Tremfya, Dupixent, Skyrizi, and more. ​

If you are using a high-cost specialty medication, a quick online search of the drug name along with “Patient Assistance Program” or “Manufacturer’s Assistance Program” can help you find eligible support programs. Please be aware that participating in these programs (or any program outside of the Highmark medical/rx plan or Rx ‘N Go) may mean your prescription drug expenses won’t count toward your Highmark medical/rx plan deductible and out-of-pocket maximum. These programs operate independently from Onvo-sponsored plans, but we want to ensure you know these options exist to help reduce your prescription drug costs.​

NEW! GREEN IMAGING Effective November 1, Onvo will partner with Green Imaging to offer an alternative option forimaging services, including X-rays, MRIs, CT scans, and CAT scans. Green Imaging works with imaging facilitiesnationwide that agree to eliminate many of the additional fees often associated with radiology services, such asradiologist fees, helping reduce the overall cost of care.​ For members enrolled in the PPO 3000 plan, Onvo will cover Green Imaging imaging services at 100%, regardless ofdeductible status. Because the HDHP 4000 and HDHP 6000 plans are paired with HSAs and are subject to IRSrequirements, Onvo will cover the full cost of Green Imaging services after the deductible has been met. Before thedeductible is met on these plans, members will be responsible for a set fee charged by Green Imaging, which is standardlylower than the cost of imaging services through other providers.​

NEW! REGENEXX Regenexx is a new benefit designed to provide an alternative to orthopedic surgery. The Regenexxprocedure uses your body’s own stem cells, which are injected into the site of an orthopedic injury to help support thebody’s natural healing process without the need for surgery. Regenexx is completely optional, and Onvo is pleased to offerit as an additional treatment option for members who are interested and eligible.​ For members who qualify, Onvo will cover Regenexx consultations and procedures at 100% under the PPO 3000 plan.Under the HDHP 4000 and HDHP 6000 plans, these services will also be covered at 100% after the deductible has been met.​

HEALTH SAVINGS ACCOUNT (HSA) Onvo’s HSA will continue to be administered through Highmark Blue Cross Blue Shield. Annual contribution limits will increase for 2027; 2027 limits are now $4,500 for self-only coverage and $9,000 for family.

DENTAL & VISION Dental and Vision coverage will continue to be offered through Guardian. There will be no changes to plan designs. There will be no increase for dental contributions. There will be a slight increase to vision contributions.

VOLUNTARY BENEFITS Voluntary benefits will continue to be offered through Transamerica. These benefits include TransDI Short Term Disability, Accident, Critical Illness, Hospital Indemnity and Universal Life w/ a Chronic Conditions rider.

NEW! WISHBONE PET INSURANCE Onvo will now provide the opportunity for you to purchase insurance for your furry friends! You may enroll your cat and/or dog in Wishbone Pet Insurance at any point throughout the year.

PAID TIME OFF (PTO) As an eligible employee at Onvo, you will also have the benefit of receiving Personal Time, Vacation, and Holidays.

RETIREMENT & TAX ADVANTAGED PLANS Onvo offers a 401(K) and matches dollar per dollar up to 4%. Onvo also offers those enrolled in a Qualified High Deductible Health Plan (QHDHP) the option to enroll in a Health Savings Account (HSA).

DIRECT DEPOSIT & iPAY Direct Deposit allows you to save time and have your paycheck deposited straight into your bank account iPay will let you see pay stubs, W2, tax info, 401(K) info, and PTO balances at your fingertips!

AT&T & HIGHMARK DISCOUNTS Employees may continue to save up to 18% on Qualified AT&T Plans if you enroll in the AT&T Signature Program! Highmark offers additional discounts on gym memberships, fitness footwear, stress reduction classes, and more!

WHO IS ELIGIBLE?

Employees who work a minimum of 30 hours per week and are at least 18 years of age are eligible to enroll in the benefits described in this guide. New Hires have a waiting period of first of the month following 60 days from date of hire.

Once your enrollment is completed, you may not make any changes to your elections unless you have a Qualifying Life Event or your hours worked per week drop below the minimum.

DEPENDENT COVERAGE

You may also cover your eligible dependents, including:

  • Legal Spouse (Same Sex included), common law, domestic partners.
  • Your eligible children up to age 26 for medical, dental and vision coverage.
  • “Children” are defined as your natural children, stepchildren, legally-adopted children, and children for whom you are the court-appointed legal guardian.
  • Physically or mentally disabled children of any age who are incapable of self-support. Proof of disability may be requested.

Onvo encourages the health and financial well-being of its employees by providing access to quality and affordable healthcare. Eligible employees have access to Onvo’s comprehensive Benefit Program. Please note that any time during the plan year, Onvo may conduct audit requesting supporting documentation on all eligible dependents.

EMPLOYEES EXPERIENCING A QUALIFIED CHANGE OF STATUS

Unless you experience a Qualifying Life Event (QLE), you cannot make changes to your benefits until the next open enrollment period. If you experience a Qualifying Live Event, please contact HR. Proof of the QLE must be submitted to HR within 30 days in order to make changes to current benefit elections.

QLEs include:

  • Marriage or Divorce
  • Birth or adoption of a child
  • Change in child’s dependent status
  • Death of a spouse, child or other qualified dependent
  • Change in employment status or a change in coverage under another employer-sponsored plan
  • Entitlement to Medicare or Medicaid
  • Other events the administrator determines to be permitted or any other applicable guidelines issued by the Internal Revenue Service (IRS)

You may request benefit changes due to a Qualifying Life Event (QLE) within Workday by following the below instructions:

1. Log in to Workday

2. Navigate to Menu > Benefits > Benefit Pay Hub

3. Click the Change Benefits button

4. Select the Qualifying Life Event type that applies to your situation and the Date New Coverage Begins. This would be the date the QLE Occurred.

5. Submit any supporting documentation as requested by HR

6. Click Submit

7. A pop-up will appear. Click Open

8. Click Let's Get Started

9. Update your information as needed
