BASIC LIFE INSURANCE AND ACCIDENTAL DEATH & DISMEMBERMENT (AD&D)

Life insurance provides financial protection for your loved ones if something were to happen to you. The life insurance benefit would be paid to your beneficiaries in the event of your death. Worldwide Golf provides all benefits-eligible employees with Basic Life and AD&D coverage through Sun Life. The AD&D benefit will equal your basic life coverage amount and, under qualifying circumstances, is paid to your beneficiary in addition to the basic life benefit.

Basic Life Benefit Amount: $25,000

Basic Accidental Death & Dismemberment Benefit Amount: $25,000

Benefits are reduced at age 65 and may reduce again in subsequent years as noted in your Certificate.

VOLUNTARY LIFE INSURANCE AND ACCIDENTAL DEATH & DISMEMBERMENT (AD&D)

While Worldwide Golf provides basic life insurance, some employees may want to purchase additional life protection. Your personal circumstances should be taken into consideration. Are you the sole provider for your household? What other expenses do you expect in the future (for example, college tuition for your child)? With Voluntary Life/AD&D through Sun Life, you are responsible for paying the full cost of coverage through payroll deductions. Please make sure to update your beneficiary information in Paycom. For the rates and full details on coverage and exclusions, please review the benefit materials housed on Paycom.

Benefit Amounts:

  • Employee: You can choose coverage from $10,000 to $500,000 in increments of $10,000, not to exceed 5 times your Basic Annual Earnings.
  • Spouse: You can choose coverage from $5,000 to $500,000 in increments of $5,000, not to exceed 100% of the employee's coverage amount.
  • Child(ren): You can choose coverage from $2,000 to $10,000 in increments of $2,000, not to exceed 100% of the employees coverage amount.

Guarantee Issue (GI):

A special enrollment opportunity applies when you are first eligible for benefits. If you apply for voluntary life insurance within your initial new hire enrollment period, you are guaranteed coverage, up to the “guarantee issue” amounts below, without providing proof of good health (called “Evidence of Insurability”).

  • Employee GI: $150,000
  • Spouse GI: $30,000
  • Dependent Child(ren) GI: $10,000

If this is NOT your first time being eligible for Voluntary Life/AD&D, you will be required to submit an Evidence of Insurability to Sun Life for approval for any coverage amount.

Voluntary Life/AD&D is available to Spouse and Dependent Child(ren) only if the employee elects coverage.

VOLUNTARY SHORT TERM DISABILITY

Worldwide Golf offers Voluntary Short-Term Disability to employees through Sun Life. Short Term Disability insurance can be referred to as "paycheck" insurance. The benefits payable are a percentage of pre-disability earnings up to a maximum per month. This coverage provides a weekly income replacement benefit. Benefits begin as soon as 8 days from the date you are unable to work due to an injury or illness and may be paid for up to 12 weeks (as long as you are still unable to work due to a covered disability).

Please note, for California employees, the weekly benefit is offset by the state of California's coverage.
