Worldwide Golf is introducing the UHC Value Plan for 2026! The Select+ PPO HSA Plan & the CA Select EPO Plan (California employees only) will continue to be offered with United Healthcare (UHC).

UHC VALUE PLAN

The UHC Value Plan provides both in- and out-of-network coverage, however you will always see the greatest coverage and savings when visiting providers within UHC's Select Plus PPO Network.

This plan has a $5,000 employee-only and $10,000 employee/dependent deductible that applies toward most medical services like an inpatient hospital stay or outpatient surgery.

In order to access the 100% coinsurance level, the deductible must be satisfied. Prescription drugs and some medical services on the Low Medical plan have flat copays, where the deductible does not apply. These are fixed dollar amounts that are paid to providers on a per visit or per prescription basis. For example, a primary care visit is a flat $50 copay and the annual deductible does not need to be satisfied. Note: Preventive care and preventive medications are covered at no cost to you and the deductible is waived when you choose an in-network provider.

UHC Value Plan Summary

SELECT+ HSA PPO PLAN (HIGH DEDUCTIBLE HEALTH PLAN WITH HSA & HRA)

The Preferred Provider Organization (PPO) Medical Plan provides both in- and out-of-network coverage, however you will always see the greatest coverage and savings when visiting providers within UHC's Select Plus PPO Network.

This medical plan has an in-network deductible of $6,350 for employee-only and $12,700 for employee/dependent coverage that applies to all medical services and prescription medications. You will be responsible for the full cost for your care until you satisfy your deductible. Once met, the plan will begin paying for 100% of the cost of your in-network medical services and prescriptions for the rest of the calendar year.

A unique pairing of this High Deductible Health Plan is the Health Savings Account (HSA) and the Health Reimbursement Arrangement (HRA). These enhanced features work together with your PPO medical plan to provide you with flexible and comprehensive medical coverage. Worldwide Golf helps offset the majority of your overall deductible by contributing to your HSA along with providing HRA funding that is available for reimbursement.

Select+ PPO HSA Plan Summary

CA SELECT EPO PLAN* (CALIFORNIA EMPLOYEES ONLY)

The Exclusive Provider Organization (EPO) Medical Plan provides in-network coverage only. The network is the UHC Select Network. There is NO out-of-network coverage with this medical plan, except for emergencies. You will no longer need a referral to see a specialist under this plan. This plan has a $1,500 employee-only and $3,000 employee/dependent deductible that applies toward select major medical services like an inpatient hospital stay or outpatient surgery. In order to access the 70% coinsurance level, the deductible must be satisfied. Most medical services and prescription drugs on the CA Select EPO plan have flat copays, where the deductible does not apply. These are fixed dollar amounts that are paid to providers on a per visit or per prescription basis. For example, a primary care visit is a flat $25 copay and the annual deductible does not need to be satisfied. Note: Preventive care and preventive medications are covered at no cost to you and the deductible is waived when you choose an in-network provider.

  • The HDHP in-network deductible is $6,350 for employee-only coverage and $12,700 for employee/dependent coverage.

  • Once the in-network deductible is satisfied, the plan will begin paying 100% toward in-network medical and prescription expenses.

HEALTH SAVINGS ACCOUNT (HSA) (Select+ HSA PPO Plan Only)

  • The Health Savings Account is a portable bank account that allows you to contribute pre-tax dollars to pay for qualified health care expenses. The HSA can be used to help pay for medical and prescription expenses that apply toward the HDHP deductible.
  • On top of any pre-tax contributions you contribute, Worldwide Golf also contributes $10 per paycheck for individuals or $20 per paycheck if you cover dependent(s) on your plan. You can contribute up to the 2025 IRS maximum contribution limits, which is inclusive of Employee and Employer contributions.

Please note: You own your HSA and the funds in the account are yours to keep - even if you leave the company.

HEALTH REIMBURSEMENT ARRANGEMENT (HRA) (Select+ HSA PPO Plan Only)

  • The HRA is solely funded with Worldwide Golf's dollars, which is provided to help you pay for out-of-pocket medical and prescription expenses before meeting your deductible.
  • Employee Only Coverage: Once you accumulate $2,000 of the $6,350 in-network deductible from in-network medical and prescription expenses, the HRA will begin to reimburse for medical and prescription expenses for the remaining portion of your deductible ($4,350). Once the deductible has been met, the plan will begin paying 100% toward in-network medical and prescription expenses.
  • Employee/Dependent Coverage: Once you accumulate $4,000 of the $12,700 in-network deductible from in-network medical and prescription expenses, the HRA will begin to reimburse for medical and prescription expenses for the remaining portion of your deductible ($8,700). Once the deductible has been met, the plan will begin paying 100% toward in-network medical and prescription expenses.

PRESCRIPTION DRUG

When you enroll in one of the available medical plans, you will automatically be provided with prescription drug benefits. Worldwide Golf's Prescription Drug Program is administered through United Healthcare.

Your prescription drug benefits include different pricing structures or “tiers” that enable you to control costs based on the medications you select. Therefore, selecting a generic medication over a brand formulary medication will result in paying a lower copay. MAIL ORDER

Mail order is available with your medical/prescription drug plan. You can use mail order to get a 90-day supply of maintenance medications for a lower cost than if you went to a retail pharmacy.

United Healthcare's Virtual Care benefit provides a convenient way to access quality care. The virtual care option is in addition to your traditional network of providers. UHC Virtual Visits is available to employees and dependents enrolled in a Worldwide Golf medical plan.

You and your dependents can avoid unnecessary visits to the Emergency Room and/or Urgent Care and long waits for doctor appointments by utilizing UHC Virtual Visits.

Note: Your Primary Care Physician and other doctors may also offer phone or video call visits. When you have a telehealth visit with your regular doctor or specialist outside the Virtual Visits platform, you would likely pay the same copay as you would for an in-person visit. Review your plan’s benefit summary to learn more.

Virtual Visits are available at no cost on the EPO plan and $50 or less on the Select+ HSA PPO & UHC Value Plans. You have access to licensed physicians for common illnesses and common minor medical conditions such as:
