Patient Care Medical Group will continue to offer the Elite Specialty Option for dental, vision, and basic life insurance coverage through Oxford Benefit Management (OBM).

The OBM Elite Specialty Option provides dental services and vision benefits to employees and their dependents. This option also includes basic employee life coverage.

Under the OBM Elite Specialty Option, you and your dependents have access to United Healthcare dental and vision providers.

DENTAL

DENTAL PLAN

Patient Care Medical Group will continue to offer the Dental PPO plan through Oxford Benefit Management (OBM). Under this plan, you will have access to both in-network and out-of-network dental providers. However, please keep in mind that receiving care from in-network providers is generally more cost effective.

VISION PLAN

Vision insurance can help you maintain your vision as well as detect various health problems.

Patient Care Medical Group's vision insurance through OBM entitles you to specific eye care benefits. Our policy covers routine eye exams and the purchase of materials, including eyeglasses and contact lenses.

LIFE

BASIC LIFE INSURANCE

Patient Care Medical Group will continue to offer all eligible full-time employees basic life coverage. This coverage has a flat benefit amount of $25,000.

This coverage is for employees only. Dependents are not eligible.
