Wellness Credit
PFN’s Wellness Program is designed to help you manage and improve your well-being. Eligible employees and their spouses will qualify for Level 1 by completing one of the categories listed below. Employees and spouses will qualify for Level 2 by completing all of the categories listed below.
- Dental Cleaning and/or Exam
- Annual Physical (Pre-employment doesn’t count)
- Biometrics
New Team Members will have 90 days to complete the Wellness requirements to reach their levels.
The deadline to complete your wellness requirements in order to receive the 2025 Wellness Contributions is 11/22/2024. These 2025 Wellness Contributions will begin effective the first payroll of 2025.
Wellness Contribution Credit
- Level 1 Single - EE, EE+Child(ren) coverage: $468 annual credit
- Level 1 Family - EE+ Spouse, Family coverage: $936 annual credit
- Level 2 Single - EE, EE+Child(ren) coverage: $910 annual credit
- Level 2 Family - EE+ Spouse, Family coverage: $1,820 annual credit
SERVICE BANDED MEDICAL CONTRIBUTIONS
PFN has decided to continue using service banded medical contributions. You will continue to be rewarded for the longer you stay at PFN! The service bands are as follows:
- 0 - 2 years served at PFN
- 3 - 10 years served at PFN
- 11+ years served at PFN
Anyone who will reach the next service band in 2025, will transition effective 01/01/2025 and must make an election to their medical plan during the open enrollment period.
