
Benefits are a valuable part of your overall compensation. The Chefs' Warehouse is proud to offer valuable benefits to you and your family. They can help protect important things like your income and assets if you become sick or injured and can't work. Some insurance products can help you pay for expenses that aren't covered by your health insurance, like copays, deductibles, and other out-of-pocket expenses.
You will have the option to enroll in a Traditional Managed Care plan, or a Reference Based Pricing Plan offered through UMR/HealthScope. Through UMR, you will have access to UnitedHealthcare's (UHC) Choice Plus network of providers. Please refer to the Medical & Prescription page for more details about the plans.
If you are enrolled in the Medical and Prescription program and if you need assistance with a medical bill you do not understand, if you have questions about your medical and prescription coverage, if you need assistance with a medical or prescription claim, etc. you will continue to have access to the Graham Service Line at 888-229-6494.
BENEFITS OVERVIEW
MEDICAL & PRESCRIPTION PLANS You will have two medical and prescription plan options to choose from - The OAP and HDHP with HSA. However, you can choose to enroll in either of the plan options under a Traditional Managed Care model with access to United Healthcare's Choice Plus Network or under a Reference-Based Pricing model, with access to Multiplan's network. All plans will be administered by UMR/HealthScope for the 2024/2025 plan year. Your prescription benefits will continue to be administered by USRX care in both plan arrangements.
RX 'N GO Rx 'N Go is a mail-order pharmacy alternative with over 1,000 generic and brand maintenance medications covering conditions such as Asthma, Diabetes, Hypertension, Mental Health, and more. This benefit is only available to those enrolled in one of Chefs' medical plans. Learn more here!

(NEW!) CHECKUPS BY PATHFINDER This program offers you a preventative health visit and consultation with a doctor, virtually. CheckUps makes preventive care quick and convenient - sign up, request an at-home lab kit, and schedule a virtual appointment with your doctor. Learn more here!
TELEMEDICINE The Telemedicine benefit is administered through Teladoc. This benefit is available to all Chefs' medical & prescription plan enrollees.
DENTAL PLANS You will continue to have two dental plans to choose from: the DPPO and the DHMO. The dental plans will continue to be administered by Cigna. NEW - Cigna Dental now covers Composite (White/Tooth Colored) Fillings.
VISION PLANS The vision plan will continue to be administered by EyeMed. There are no changes to the plan designs.
SPENDING ACCOUNTS The Healthcare, Dependent Care, and Commuter Flexible Spending Accounts will continue to be administered by TASC. Note, there will be a separate open enrollment held in November, if you would like to participate in these benefits.
RETIREMENT The 401(k) plan will continue to be administered by Principal.
LIFE AND DISABILITY The life and disability plans will continue to be administered by MetLife.
(NEW!) VOLUNTARY The Chefs' Warehouse will continue to offer workplace voluntary coverages including Accident, Critical Illness, Voluntary Long-Term Disability, and a NEW Hospital Indemnity Plan. These coverages will be administered by MetLife. Please see the Voluntary Benefits page to see more! The Universal Life policy will continue to be offered through Transamerica. The Legal Protections and Identify Theft coverage will continue to be offered through Legal Shield. The Pet Insurance will continue to be offered through Nationwide.
EMPLOYEE ASSISTANCE PROGRAM (EAP) The EAP resource will continue to be offered by The Chefs' Warehouse through MetLife.
HEALTHJOY HealthJoy is an employee benefit and healthcare guidance app that helps employees make better healthcare decisions while also improving their healthcare experience. HealthJoy is a free benefit for all employees enrolled in a medical plan.
ENROLLMENT CONSIDERATIONS
This Open Enrollment is a Passive Enrollment. This means that you ONLY need to log into Ceridian Dayforce to complete your enrollment if you want to make any changes to your current elections. If you do not wish to change your current elections, your 2024-2025 elections will roll over into the new plan year. Spending Account elections (HSA/FSA) will be required to be selected in a separate enrollment, as these benefits run on a calendar year basis and will become effective January 1, 2025. All other benefit elections made during open enrollment will go into effect on October 1, 2024. Please note that if you do not enroll within the open enrollment period, you will only be allowed to make changes during the next open enrollment period or by experiencing a valid qualifying life event. Employees are responsible for reviewing their benefits and pay stubs and reporting any discrepancies.
The Chefs’ Warehouse provides certain benefits on a shared contribution basis and voluntary plans on an employee-paid basis. This benefit brochure is intended to highlight features of the benefits offered to The Chefs’ Warehouse employees and may not reflect all benefits offered. In the event there is a contradiction between the information in this brochure and the information contained in the Master Contracts or Master Plan Documents with the insurance carriers, the Master Contracts and Plan Documents will govern.

Open Enrollment is the period each year when you can make changes to your benefits. This guide will outline benefit options that are available to you and your family.
WHO IS ELIGIBLE?
Full time employees of The Chefs’ Warehouse become eligible for benefits on the first day of the month following the date of hire.
To be considered a benefits eligible employee, you must average 30 hours per work week.
DEPENDENT COVERAGE
In addition to electing coverage for yourself, you can elect to cover your eligible dependents. Your eligible dependents include your legal spouse*, domestic partner*, and dependent children up to age 26 regardless of student status, marital status, financial dependence or residence and your children of any age who are mentally or physically disabled and depend on you for support.
EMPLOYEES EXPERIENCING A QUALIFIED CHANGE OF STATUS
Unless you experience a life-changing qualifying event, you cannot make changes to your benefits until the next open enrollment period.
Qualifying events include:
- Marriage, divorce or legal separation
- Birth of your child
- Death of your spouse or dependent child
- Adoption of or placement for adoption of your child
- Award of legal guardianship
- Change in employment status of employee, spouse, or dependent child
- Qualification by the Plan Administrator of a child support order for medical coverage
- Entitlement to Medicare or Medicaid
You must enroll within 30 calendar days of the qualified life event. Depending on the type of event, you may be asked to provide proof of the event. You may also add new dependents to your existing coverage within 30 calendar days of the qualifying life event. If you do not make the change within 30 calendar days of the qualified event, you will have to wait until the next annual enrollment period to make changes.
You may add new dependents to your existing benefits within 30 calendar days of the qualifying life event.
