BASIC LIFE INSURANCE - EMPLOYER PAID

Life insurance can help provide for your loved ones if something were to happen to you. The Chefs' Warehouse provides all regular, full-time employees working a minimum of 30 hours per week with up to $300,000 in group life and accidental death and dismemberment (AD&D) insurance.

SHORT-TERM DISABILITY INSURANCE - EMPLOYER PAID

What would you do if a sickness or injury left you unable to come to work and earn your paycheck? In the event that you become disabled from an injury or sickness, disability income benefits will provide a partial replacement of lost income.

The Chefs' Warehouse provides Short-Term Disability at no cost to you.

SUPPLEMENTAL LIFE INSURANCE - EMPLOYEE PAID

While The Chefs' Warehouse offers basic life insurance, some employees may want to purchase additional coverage.

This is about your personal circumstances. Are you the sole provider for your household? What other expenses do you expect in the future (for example, college tuition for your child)? Depending on your needs, you may want to consider buying voluntary life insurance.

With voluntary life insurance, you are responsible for paying the full cost of coverage through payroll deductions. You can purchase coverage for yourself or for your dependents.

Employees electing Voluntary Life and AD&D coverage for the first time, or if you make a request during an annual enrollment period to increase the amount of your Supplemental Life Insurance by more than one level above your current amount of Supplemental Life Insurance ($10,000), EOI will be required.
