As a UMR/HealthScope enrollee, you have access to resources that will help you navigate and maximize your benefits.

UMR MEMBER RESOURCES

As a UMR/HealthScope medical enrollee, you will have access to the member portal. The member portal will permit you to view your coverage, deductible, out-of-pocket maximum accumulators, claims, locate a provider, and more.

You will need your UMR/HealthScope ID card in order to register your account for the first time. Please click the "Welcome to HealthScope" button below to learn more.

Click the link below to access the member portal. Note, you will be required to provide your Member ID number (located on your ID card) to register and access your account. Once you enter your Member ID, you will see The Chefs' Warehouse Company Name present.

Locating a Provider: Reference Based Pricing Arrangement

Below you will find comprehensive instructions on how to locate a provider. While you can utilize any provider you choose under the Reference Based Pricing arrangement, there are certain providers that are participating in the PHCS (Multiplan) network that are preferred.

Locating a Provider: Traditional Managed Care Arrangement

Should you choose to enroll in one of the plans under a Traditional Managed Care arrangement, you will have access to UHC's Choice Plus network of providers. Keep in mind, your benefits will be maximized under this arrangement by utilizing in-network providers and facilities. To locate in-network providers, please follow the link below.

WHAT TO DO IF YOUR PROVIDER DOES NOT RECOGNIZE YOUR INSURANCE

If your provider indicates that they do not accept UMR/HealthScope insurance, please instruct your provider to call the HealthScope phone number on the back of your ID card to verify your eligibility for benefits. A HealthScope Service Team member will explain your medical coverage to your provider.

At any time, if you are asked to pay up front, immediately call HealthScope to speak to someone who will work through the issue right away. It is also recommended you contact UMR/HealthScope ahead of receiving care to ensure you receive the appropriate care and understand your benefits.

Gym Reimbursement Program

If you visit the gym regularly, you may be eligible for gym reimbursement through UMR/HealthScope! In order to receive reimbursement, you must be enrolled in a United Healthcare/UMR medical plan through The Chefs’ Warehouse, visit a gym 50 times within a 6-month period, and complete and submit the claim form below

What is the Reimbursement Amount?

Maximum payout for gym reimbursement is $200 per 6-month period for employees and $100 for the spouse per 6-month period.

The maximum payout for Peloton is $200 per calendar year for employees, and $100 for the spouse or dependent per calendar year.

The calendar year is from January 1, 2024 – December 31, 2024
