CastleWorks is thrilled to be able to offer five new comprehensive medical plans, administered by UnitedHealthcare for the January 1, 2025 - December 31, 2025 plan year.

SV HARMONY HMO

The SV Harmony HMO plan requires you to choose a primary care provider (PCP) who will coordinate the overall medical care for you and your covered dependents. Your PCP is the doctor that will treat you for your basic health care needs. Anytime you need to see a specialist, your PCP will refer you to a specialist participating in the network. You are required to get a referral from your PCP for specialty services. All referrals are done electronically. You won’t need a referral for OB/GYN, Physician Services and Emergency/Urgently Needed Services.

There is a $2,000 deductible for individuals and a $4,000 deductible for family tiers. Co-pays and coinsurance for physicians, hospital, and/or Emergency Room visits apply, and do not require you to hit the deductible first. There is no coverage for out-of-network services.

The HMO SV Harmony is the lowest costing of the three UHC medical HMOs due to the smaller size of it's network. You can see what providers participate in the Harmony Network by clicking the Harmony Network Info button below.

SV ALLIANCE HMO

The SV Alliance HMO plan requires you to choose a primary care provider (PCP) who will coordinate the overall medical care for you and your covered dependents. SELECT PLUS

The Select Plus plan has a $4,000 deductible for individuals and a $8,000 deductible for family tiers. Co-pays for physicians, specialists, and urgent care do not require you to hit the deductible first. Major medical services such as inpatient hospital stays and emergency services are covered at a coinsurance percentage after having hit the deductible.

This Medical Plan offers the flexibility of receiving care from in-network and out-of-network providers. However, utilizing in-network providers will maximize your medical benefit.

SELECT PLUS HSA

The Select Plus HSA plan has a $7,000 deductible for individuals and a $14,000 deductible for family tiers. All medical services are covered at 100% after hitting the deductible.

When enrolled in the Select Plus HSA, you may contribute to a Health Savings Account (HSA). HSAs are tax-advantaged savings accounts designed to help individuals save for qualified medical expenses. Contributions to an HSA are made with pre-tax dollars, reducing taxable income, and funds can grow tax-free. Withdrawals for qualified medical expenses, such as doctor visits, prescriptions, and certain medical procedures, are also tax-free. HSAs offer flexibility, as unused funds can roll over from year to year, and account holders can invest their savings for potential growth. Click here to read more about HSAs!
