To help you manage your health and well-being, Graham offers three comprehensive, high quality medical options that include prescription drug coverage. Choosing your medical benefits depends on your specific health care needs, doctor preferences and budget. Each option was designed to protect you and your family from major financial hardship in the event of an illness or injury.

WHO IS ELIGIBLE?
Full time employees are eligible upon date of hire.
WHAT NETWORK DO WE USE?
Graham utilizes the Aetna Choice POS II Network through Meritain Health, an Aetna Company.
ARE PLANS CHANGING?
Per IRS guidelines, the deductible on medical Plan Option 1, formerly known as the 1600/3200 HSA Plan, will be increasing from $1,600 to $1,650 for individual coverage and from $3,200 to $3,300 for family coverage.
Per IRS guidelines, the deductible on medical Plan Option 2, formerly known as the 3200/6400 HSA Plan, will be increasing from $3,200 to $3,300 for individual coverage and from $6,400 to $6,600 for family coverage.
Other than these changes, plan designs will remain the same.
SHORT PLAN YEAR
During the Short Plan Year, you will still be subject to the full medical plan deductible during the six-month period (per IRS requirements). Your deductible will reset for the new plan year beginning January 1, 2026. Please consider this when making your plan selection for July 1.
PROGRAM ENHANCEMENTS & REMINDERS
Goldfinch Health: This benefit is designed to help you and your loved ones in anticipation of an upcoming surgery.
Healthcare Bluebook: Graham employees enrolled in our Medical Plan have access to the HCBB platform to view quality ratings on hundreds of procedures across thousands of hospitals nationwide. We will also be incorporating a Rewards Program for the 2025 plan year.
Nutritional Counseling: Meritain covers 10 annual visits for coaching. Please contact Meritain's Member Services to find a participating provider or ask Human Resources for more information on the Charge Group.
HealthJoy: Graham benefit eligible employees will have instant access to an up-to date benefits wallet with all of your benefits cards. HealthJoy's healthcare concierge is available to help you as well.
Regenexx: Regenexx is a new approach to orthopedic care. This procedure is an alternative to surgery and is designed to promote your body’s natural healing abilities to treat the injury and avoid surgery.
OPTION 1: 1650/3300 HSA
This plan is a great choice for employees who may not utilize coverage often and have interest in a Health Savings Account. If you choose to enroll in this plan, all services will apply to your deductible. If you enroll in this plan, you have the option of enrolling in an HSA to help with the out-of-pocket cost. OPTION 2: 3300/6600 HSA
This plan is a great choice for employees who may not utilize coverage often with a $0 cost premium option. However, it is important to note the increased employee responsibility as there is a larger deductible. If you choose to enroll in this plan, all services will apply to your deductible. You have the option of enrolling in an HSA to help with this out of pocket cost. You can read more about the HSA here.
ABOUT YOUR PRESCRIPTION DRUG COVERAGE
With your prescription drug coverage, all medications or alternatives will be listed on CVS's formulary. How much you need to pay for your medications can vary on what tier your medication is, and on which medical plan you are enrolled in. Please see the below for access to CVS's formulary. The medications listed at the top of the formulary are included. Please note that the formulary is subject to change at any time throughout the plan year.
You will then see CVS's "Preferred Options List". This outlines non-formulary medications and the preferred alternative on the formulary. l medications will be classified under the following definitions:
Generic Medication: These have the same chemical make-up, safety, strength, and route of administration as name-brand. Generics are often the cheapest drug alternative.
Formulary medication: This includes drugs that are listed under CVS's formulary. This can include generic and name-brand drugs.
Non-Formulary Medication: This means that your drug is not on the formulary, or is excluded. The formulary will also have other alternative available if you are willing to move away from the specific brand you are used to.
WEIGHT MANAGEMENT PROGRAM
Graham employees and dependents enrolled in one of the Meritain medical plan options have access to the CVS Weight Management Program through the Health Optimizer App. This 12-month program is a holistic approach to weight loss that includes one-on-one support from a dedicated registered dietitian (RD) for months one through six, a connected scale and other devices based on clinical needs, provider oversight to adjust or prescribe weight loss medication as appropriate, and a host of online resources to aid you in your journey.
Please note that you are required to complete the weight management program for six months before your physician can prescribe weight loss medication. This allows members to explore all options and set a healthy foundation before beginning medication, as true lifestyle changes are what maintain weight loss results in the long term.
