This guide was designed to answer some of the basic questions you may have about your benefits. Please read it carefully along with any supplemental materials you receive.
Please note that this year's open enrollment is PASSIVE, meaning that all current benefit elections other than Health Savings Account (HSA) contributions will automatically carry over into the 2025-2026 plan year. If you wish to make changes to your previous benefits, add new ones, waive coverage, or make contributions to your HSA for 2025-2026, you must sign into our new benefits platform, Workday. Please refer to the Enrollment Information page for instructions on how to enroll.

BENEFITS OVERVIEW
MEDICAL PLAN Onvo will continue to offer employees the option to enroll in one of three medical plan offerings through Highmark. There will be no changes to the medical plan designs.
NEW! RX N GO New this year, employees may elect to have certain maintenance medications delivered straight to their door in 90-day supplies for a $0 copay and $0 delivery fees! Navigate to the Rx 'N Go page to learn more about the program and confirm whether your prescription(s) are covered.
HEALTH SAVINGS ACCOUNT (HSA) Onvo’s HSA will continue to be administered through Highmark Blue Cross Blue Shield. Annual contribution limits will increase for 2026; 2026 limits are now $4,400 for self-only coverage and $8,750 for family.
DENTAL & VISION Dental and Vision coverage will continue to be offered through Guardian. There will be no changes to plan designs or employee contributions.
VOLUNTARY BENEFITS Voluntary benefits will continue to be offered through Transamerica. These benefits include TransDI Short Term Disability, Accident, Critical Illness, Hospital Indemnity and Universal Life w/ a Chronic Conditions rider.
PAID TIME OFF (PTO) As an eligible employee at Onvo, you will also have the benefit of receiving Personal Time, Vacation, and Holidays.
RETIREMENT & TAX ADVANTAGED PLANS Onvo offers a 401(K) and matches dollar per dollar up to 4%. Onvo also offers those enrolled in a Qualified High Deductible Health Plan (QHDHP) the option to enroll in a Health Savings Account (HSA).
DIRECT DEPOSIT & iPAY Direct Deposit allows you to save time and have your paycheck deposited straight into your bank account iPay will let you see pay stubs, W2, tax info, 401(K) info, and PTO balances at your fingertips!
AT&T & HIGHMARK DISCOUNTS Employees may continue to save up to 18% on Qualified AT&T Plans if you enroll in the AT&T Signature Program! Highmark offers additional discounts on gym memberships, fitness footwear, stress reduction classes, and more!
WHO IS ELIGIBLE?
Employees who work a minimum of 30 hours per week and are at least 18 years of age are eligible to enroll in the benefits described in this guide. New Hires have a waiting period of first of the month following 60 days from date of hire.
Once your enrollment is completed, you may not make any changes to your elections unless you have a Qualifying Life Event or your hours worked per week drop below the minimum.
DEPENDENT COVERAGE
You may also cover your eligible dependents, including:
- Legal Spouse (Same Sex included), common law, domestic partners.
- Your eligible children up to age 26 for medical, dental and vision coverage.
- “Children” are defined as your natural children, stepchildren, legally-adopted children, and children for whom you are the court-appointed legal guardian.
- Physically or mentally disabled children of any age who are incapable of self-support. Proof of disability may be requested.
Onvo encourages the health and financial well-being of its employees by providing access to quality and affordable healthcare. Eligible employees have access to Onvo’s comprehensive Benefit Program. Please note that any time during the plan year, Onvo may conduct audit requesting supporting documentation on all eligible dependents.
EMPLOYEES EXPERIENCING A QUALIFIED CHANGE OF STATUS
Unless you experience a Qualifying Life Event (QLE), you cannot make changes to your benefits until the next open enrollment period. If you experience a Qualifying Live Event, please contact HR. Proof of the QLE must be submitted to HR within 30 days in order to make changes to current benefit elections.
QLEs include:
- Marriage or Divorce
- Birth or adoption of a child
- Change in child’s dependent status
- Death of a spouse, child or other qualified dependent
- Change in employment status or a change in coverage under another employer-sponsored plan
- Entitlement to Medicare or Medicaid
- Other events the administrator determines to be permitted or any other applicable guidelines issued by the Internal Revenue Service (IRS)
You may request benefit changes due to a Qualifying Life Event (QLE) within Workday by following the below instructions:
1. Log in to Workday
2. Navigate to Menu > Benefits > Benefit Pay Hub
3. Click the Change Benefits button
4. Select the Qualifying Life Event type that applies to your situation and the Date New Coverage Begins. This would be the date the QLE Occurred.
5. Submit any supporting documentation as requested by HR
6. Click Submit
7. A pop-up will appear. Click Open
8. Click Let's Get Started
9. Update your information as needed
