INTRODUCTION

Steve Shannon Tire & Auto Centers strives to provide you and your family with a comprehensive and valuable benefits package. We want to make sure you’re getting the most out of our benefits – that’s why we’ve put together this New Hire Benefits Guide.

BENEFITS OVERVIEW

MEDICAL & PRESCRIPTION DRUG Steve Shannon offers employees the option to enroll in one of three medical plan offerings, administered through HealthNow (HNAS) and Optum/RxBenefits. One of the plans, the RBP 1000, is a Reference Based Pricing arrangement. The RBP 1000 plan is NOT a traditional insurance plan. If you are considering this option, we request that you please contact the Graham Concierge Team at 1-888-842-1488 or Graham-Benefits@MarshMMA.com to receive a detailed overview of the plan. Because this plan operates differently from standard insurance in certain ways, it’s important to fully understand its features before enrolling.

The PPO 2500 and PPO 1000 plans are traditional medical insurance plans that utilize the Highmark provider network. Please visit the Medical and Reference Based Pricing pages for more details.

DENTAL & VISION Dental coverage includes preventive, basic, and major care. Vision coverage includes exams, lenses, and frames. Those enrolled in medical/rx coverage are automatically enrolled in vision. Both dental and vision are administered through Highmark.

LIFE INSURANCE Steven Shannon Tire offers employees a company-paid life insurance benefit through Transamerica. Please ensure that you enter beneficiary information in Employee Navigator. You may change your beneficiaries at any point throughout the year, but the system requires that you enter at least one primary beneficiary during the new hire enrollment process.

VOLUNTARY BENEFITS As an eligible employee, you have the option to enroll in voluntary benefits, including Accident, Critical Illness, Short-term Disability, and Universal Life. These plans are administered through Transamerica. You may enroll in these benefits through Employee Navigator. These benefits are Employee-Paid.

ENROLLMENT INFORMATION All employees will need to complete their enrollment elections in the Employee Navigator Platform. Elections made during your new hire window will go into effect for the remainder of the plan year (until December 31, 2026) and cannot be changed mid-year unless a qualified life event occurs.

WHO IS ELIGIBLE?

Employees who work a minimum of 30 hours per week and are at least 18 years of age are eligible to enroll in the benefits described in this guide. New Hires have a waiting period of 90 days from date of hire.

Once your enrollment is completed, you may not make any changes to your elections unless you have a Qualifying Life Event or your hours worked per week drop below the minimum.

DEPENDENT COVERAGE

You may also cover your eligible dependents, including:

  • Legal Spouse.
  • Your eligible children up to age 26 for medical, dental and vision coverage.
  • “Children” are defined as your natural children, stepchildren, legally-adopted children, and children for whom you are the court-appointed legal guardian.
  • Physically or mentally disabled children of any age who are incapable of self-support. Proof of disability may be requested.

EMPLOYEES EXPERIENCING A QUALIFIED CHANGE OF STATUS

Unless you experience a life-changing qualifying event, you cannot make changes to your benefits until the next open enrollment period.
