The Modern Group, Ltd. is pleased to offer you and your family a comprehensive benefits program. Understanding your healthcare options and determining the best plan options that fit your needs should be a high priority during the Annual Open Enrollment Period. We understand that navigating through your benefits package can be complex and wish to offer as much support and valuable information as possible.
BENEFITS OVERVIEW
MEDICAL PLAN Modern Group, Ltd. will continue to offer you and your dependents three Medical & Prescription Drug (Rx) plans from Aetna.
DENTAL & VISION You and your dependents will continue to have the option to enroll in dental and vision coverage. Both of these plans will be administered through Aetna.
SPENDING ACCOUNTS Modern offers employees enrolled in any of the three Aetna medical plans the opportunity to enroll in a Health Reimbursement Arrangement. Employees may also enroll in Health Care or Dependent Care Flexible Spending Accounts.
LIFE & DISABILITY Eligible employees receive basic life, short-term disability, and long-term disability coverage at no cost. Employees will also have the option to enroll in employee-paid supplemental life coverage. All Life & Disability coverages will be administered by Equitable.
ADDITIONAL RESOURCES Employees have access to a list of additional resources beyond their basic medical, dental, and vision plans.
ENROLLMENT CONSIDERATIONS
Open Enrollment for the June 1, 2023 — May 31, 2024 plan year will take place from April 26 — May 12, 2023.
Elections you make during open enrollment will become effective on June 1, 2023. If you have questions about any of the benefits mentioned in this guide, please don’t hesitate to reach out to your Human Resource team or The Graham Company’s Service Line. Open enrollment is the period each year when you can make changes to your benefits. This guide will outline benefit options that are available to you and your family.
WHO IS ELIGIBLE?
Active, Full-Time (30+ hours per week) employees of Modern Group, Ltd. may be eligible for benefits. Please refer to the SPDs for each benefit to confirm whether you and your spouse and dependents are eligible.
DEPENDENT COVERAGE
In addition to electing coverage for yourself, you can elect to cover your eligible dependents. Your eligible dependents include your spouse/domestic partner, your children up to age 26 regardless of student status, marital status, financial dependence or residence and your children of any age who are mentally or physically disabled and depend on you for support.
EMPLOYEES EXPERIENCING A QUALIFIED CHANGE OF STATUS
Unless you experience a life-changing qualifying event, you cannot make changes to your benefits until the next open enrollment period— each June 1st.
Qualifying events include:
- Marriage or Divorce
- Birth or adoption of a child
- Change in child’s dependent status
- Death of a spouse, child or other qualified dependent
- Change in employment status or a change in coverage under another employer-sponsored plan
- Entitlement to Medicare or Medicaid
