BENEFITS OVERVIEW
MEDICAL PLANS You will continue to have three medical options to choose from. The medical plans will continue to be administered by Highmark and you will continue to have access to the Highmark network. The medical plan designs will be changing, however, PFN is happy to share medical payroll contributions will not be increasing. Note, the deadline to submit Wellness Credit Forms is 11/22/2024 to receive credit for 2025. To learn more about the medical plans, click here.
NEW! SMITHRX The prescription drug program, previously run through Highmark, will now be run through SmithRx. SmithRx works hand in hand with Highmark to provide seamless integration between your medical plan and prescription drug program. There are some new features with the SmithRx program that can help lower your drug cost. You will receive a new separate SmithRx ID card! More information on SmithRx can be found on this page.
RX 'N GO PFN will continue to offer $0 cost, 90-day supply of generic and brand name medications through Rx 'n Go for FREE! To learn more about Rx 'n Go and the medications you can receive at $0 cost, click here.
SPRING HEALTH PFN is excited to continue to offer Spring Health, a robust mental health and EAP benefit available to all employees, spouses, and dependents. It is important for individuals to add/update household dependents to your "contacts" page during open enrollment so that they will be covered under this benefit. DENTAL PLANS Your dental coverage will continue to be administered through Guardian. There are no changes to the dental plan design or contributions. You will continue to have the ability to rollover part of your unused annual maximum into a Maximum Rollover Account to be used in future years. To learn more about the dental plan, click here.

LIFE AND DISABILITY PFN provides full-time employees with Basic Life/AD&D insurance, Short-Term Disability insurance, and Long Term Disability insurance at no cost to you! You will also continue to have the opportunity to purchase Supplemental Life insurance for yourself, your spouse, and your dependents. SPENDING ACCOUNTS PFN will continue to offer the Healthcare Care Flexible Spending Account to all full-time employees and the Health Savings Account to those enrolled in PPO Option 3. VOLUNTARY BENEFITS PFN is pleased to continue to offer Accident insurance, Critical Illness insurance, and Hospital Indemnity insurance. These voluntary benefits will be administered through Guardian. To learn more about the voluntary benefits, click here.

ENROLLMENT CONSIDERATIONS
Benefit elections you make during open enrollment will become effective on January 1, 2025. If you have questions about any of the benefits stated in this guide, please do not hesitate to reach out to Human Resources, or The Graham Company Service Line at 1-888-842-1488. This guide will outline benefit options that are available to you and your family.
WHO IS ELIGIBLE?
All active, full-time employees working 30 or more hours per week are eligible to participate in the PFNonwovens benefit program.
DEPENDENT COVERAGE
In addition to electing coverage for yourself, you can elect to cover your eligible dependents. A dependent child may be the natural child, stepchild, legally adopted child, a child placed for adoption, or other children for whom you have permanent legal custody. An eligible child may participate in the benefits under the plan until age 26.
EMPLOYEES EXPERIENCING A QUALIFIED CHANGE OF STATUS
Unless you experience a life-changing qualifying event, you cannot make changes to your benefits until the next open enrollment period.
Qualifying events include:
- Marriage or Divorce
- Birth or adoption of a child
- Change in child’s dependent status
- Death of a spouse, child, or other qualified dependent
- Change in employment status or a change in coverage under another employer-sponsored plan
- Entitlement to Medicare or Medicaid

ADDITIONAL BENEFITS
HOURLY PAID TIME OFF
Holidays - Ten (10) paid Holidays per calendar year. Personal Time Off - Three (3) paid days per year - after 90 days. Vacation - Eligibility based on the following schedule:

MISCELLANEOUS BENEFITS
On-site Fitness Center (if available) – eligibility to use the on-site facility begins at date of hire after completing a brief orientation. As at the date hereof, the facility is equipped with top of the line cardio and weight training equipment, and provides locker rooms for your use. There is no cost for use of the Fitness Center. Please be advised that the Company reserves the right to discontinue the On-site Fitness Center at any time, acting in its sole and absolute discretion.
On-site Cafeteria – the cafeteria provides hot meal service at designated meals times. As at the date hereof, options include hot and cold sandwiches, pizzas, salads, and a different full course meal each day. Please be advised that the Company reserves the right to discontinue the On-site Cafeteria at any time, acting in its sole and absolute discretion.
QUARTERLY INCENTIVE PLAN
In addition to the 401k, PFNonwovens also offers a Quarterly Incentive Plan. You are eligible to participate beginning the first full Quarter after your date of hire. If the quarterly objectives have been met, then you are eligible for a maximum of 1.25% of pay each quarter for a total maximum annual award of 5%. To be eligible for the quarterly incentive, the team member must be employed at the end of the calendar quarter.
Example: $20,000 Annual Salary x 1.25% (100% Goals Met in Quarter)
