Hamilton Lane is happy to offer three medical and prescription drug plans administered by Cigna. To decide which plan is best for you, consider the medical needs of you and your family and how much you usually spend on health care each year. It is also important to factor in how much you are willing/able to contribute for coverage via payroll deductions.
WHO IS ELIGIBLE?
Employees and their dependents (spouse/domestic partner and children) are eligible upon date of hire.
SELECTING THE PLAN THAT IS BEST FOR YOU
Cigna Traditional PPO
The Cigna Traditional PPO features no in-network deductible.
Eligible services are covered at 100%, unless assigned a specific copay. A copay is the set fee you have to pay out-of-pocket for certain services, such as a doctor’s office visit or a prescription drug.
Please remember that it is always best practice to obtain care from in-network providers to maximize your benefits.
Cigna HDHP $2,000/$4,000
The Cigna HDHP $2,000/$4,000 has an in-network individual deductible of $2,000 and an in-network family deductible of $4,000.
Once your deductible has been satisfied, the plan will cover 100% of eligible expenses.
Please remember that it is always best practice to obtain care from in-network providers to maximize your benefits.
Cigna HDHP $3,000/$6,000
The Cigna HDHP $3,000/$6,000 has an in-network individual deductible of $3,000 and an in-network family deductible of $6,000.
Once your deductible has been satisfied, the plan will cover 90% of eligible expenses. You will be responsible for paying the remaining 10% of those expenses.

Remember, you pay less out of pocket when you get health services from providers that are in-network for your Aetna health plan, even when you have out-of-network benefits.

Access to healthcare is now even easier with MDLIVE
When your primary doctor isn't available, MDLIVE is available 24/7/365 for treatment of acute conditions.
MDLIVE
MDLIVE is a telehealth provider available to you through your Cigna medical plans. Like your Primary Care physician, the board-certified physicians available through MDLive are equipped to handle routine and minor illnesses or injuries. These are conditions that are not considered emergencies but still require medical attention.
You and your dependents can avoid unnecessary visits to the Emergency Room and/or Urgent Care and long waits for doctor appointments by utilizing MDLIVE.
MDLive physicians are available on weekends and holidays, making telehealth a good option for patients seeking care outside of their PCP's operating hours. MDLIVE is also a good option for those suffering from a minor illness/injury that:
- Have limited transportation options
- Have a tight schedule
- Have experienced similar symptoms in the past
- Have a contagious illness (cold, flu, etc.)
- Do not want to leave the house (due to fatigue, child care needs, etc.)

PRESCRIPTION DRUG
Rx benefits through Cigna + Express Scripts
Each of the Cigna medical plan options include comprehensive prescription drug coverage. The prescription drug benefits included in the Cigna medical plan options are subject to the Cigna Advantage Formulary. This formulary is developed by Express Scripts. A formulary is a list of generic and brand-name drugs that your plan covers.
Each drug covered under your plan will fall under a different level or tier. The lower the tier, the lower the price - and vice versa. Each of the prescription drug coverages under the Cigna medical plans have three tiers:
1. Preferred Generic - Generic drugs have the same active ingredients and work the same way as the brand-name drugs they copy.
They usually cost less than the brand-name versions.
2. Preferred Brand - These drugs are included in a plan’s list of covered drugs and may not have a generic version. They cost more
than generic drugs but less than non-preferred brand drugs.
3. Non-Preferred Brand and Generic - You will pay more for these drugs than preferred brand or preferred generic drugs.
There are several ways to save money through your prescription drug plan:
Step Therapy
Some drugs prescribed by your doctor will require step therapy. This means that you must try one or more equally effective, prerequisite drug(s) before a step therapy drug is covered. You may need to pay the full cost of the drug if the alternative(s) are not tried first.
Voluntary Mail Order
Opt into the mail order program for direct delivery of your maintenance medications used to treat chronic conditions like asthma, diabetes, and high cholesterol. You can locate the mail order form on myCigna or call Cigna One Guide to enroll.
Specialty Pharmacy
Members utilizing specialty drugs must have these prescriptions filled through Cigna Specialty Pharmacy. Members are allowed a one-time transition fill at any retail or specialty pharmacy, but then must utilize Cigna Specialty Pharmacy for any refills moving forward.
Mandatory Generic
When you request a brand drug, you pay the brand cost share plus the cost difference between the brand and generic drugs. This does not apply if the physician indicates "Dispense As Written" (DAW). Try your best to opt for the generic version of a drug to decrease your costs.
Cigna 90 Now Program
For specific maintenance medications, you are required to obtain a 90-day prescription for the medication to be covered by the plan - whether it be filled at a retail location or via mail order. Otherwise, you will pay the entire cost of the drug after you receive three 30-day fills.
